Some alternative treatments for sciatica can include acupuncture or chiropractic care. Acupuncture is typically safe and in some cases, very effective. Chiropractic care can be effective, but it would be better if chiropractic care is initiated after an MRI is obtained. Any significant manipulation of the lumbar spine that’s forceful has the risk of worsening a disc herniation and actually worsening the sciatica symptoms. So in general, I advise patients to have an MRI first to make sure there’s not a huge disc herniation pushing on the nerve before partaking in activities or in treatments that impart significant force or significant manipulation onto the lumbar spine. Simple massage, stretching, or strengthening of the core muscles of the spine is usually very helpful, and as long as there’s no significant manipulation of the lower back, should be very safe.
